You may print and completly fill out this form and then either fax back or present it at the time of rental along with a copy or actual drivers liscense of the guardian signing for you. This is used for any person who is under the age of 18 and needs to have a parent or legal guardian sign the paperwork. Note a Legal Guardian must be 25 years of age or older.
