This form must be printed and faxed back or presented at time of rental. This is used for any person who is under the age of 18 and needs to have a parent or legal guardian sign the paperwork. This form must be filled out completely and a copy of the Parent or Legal Guardian's drivers license must be sent back. Note a Legal Guardian must be 25 years of age or older.
